Useful Claims Forms
Electronic completion of claims report forms:
- Click on the document you require
- Select the ‘save’ option
- Find the ‘folder’ where you would like to save your document and double click to enter that folder. Then name your document
- Click on ‘save’ once all the above has been completed.
- When the download complete box appears select the option ‘open’
- This document has been protected so that only relevant parts of it can be changed. Because of this, you will need to click on the ‘read only’ button
- Once in the document, you will see the grey field areas that you are required to complete. Type in your answers then use your ‘tab’ key to move to the next field. Please ensure ALL questions are completed (ie all grey field areas are filled) before returning the form
- Save your document regularly to ensure you do not lose the information
- Once you have completed the form, please email it to the given email address shown at the top of the form
